Transitions to Adult Services – New Webinar Available

SENDiass4BCP are pleased to share a new webinar, created to support young people with special educational needs and disabilities (SEND), and their families, as they prepare for the transition from Children’s Services to Adult Services in Health and Social Care.

This short webinar (just over 9 minutes long) is presented by Sally from our team. She goes into the eight overarching principles that underpin a good transition, including the importance of involving young people in planning their own journey.

The webinar covers:

  • What the transition process should focus on
  • The role of a key worker in supporting and guiding the young person
  • The importance of clear information and advice throughout the process
  • What happens if a young person does not engage with Adult Health and Social Care services, and what support may still be available to meet their needs

We hope this resource will help families feel more informed and confident about what to expect during this important stage.
